Melatonin Sets the Clock, Not the Switch

Most people take melatonin like a switch that turns sleep on. It is closer to a message: the body's own signal of darkness. What it does, what it does not, and why the label so often misses.

What melatonin actually is

Melatonin is a hormone your body already makes. Doses from roughly half a milligram to five milligrams worked similarly well, itself a clue about what melatonin is doing.

The pattern holds for other misalignments. In a delayed sleep phase, where the clock runs hours late, small evening doses can help shift it earlier. For some shift workers, melatonin can ease the daytime sleep their bodies resist. The win is the same: not sedation, but a clock nudged back toward the day it is actually living in.

For ordinary sleeplessness, the story shrinks

Away from misalignment, the effect gets smaller. For garden-variety trouble falling asleep, melatonin does something, but not much. A 2013 meta-analysis of nineteen trials in people with primary sleep complaints found that, on average, melatonin helped them fall asleep about seven minutes faster than placebo and added roughly eight minutes of total sleep. Real and modest, smaller than standard sleep medications show, though the effect did not fade with continued use.

Seven minutes is not nothing, but it is not the transformation the marketing implies. A 2017 clinical practice guideline from the American Academy of Sleep Medicine weighed the same evidence and suggested against melatonin for ongoing insomnia, calling the support weak, a comment on evidence strength, not a verdict on any single night. For chronic sleeplessness that has nothing to do with timing, melatonin is a mild tool, not the answer the bottle implies.

The dose story, and the label problem

Which brings us to the number on the label, and the strangest fact about melatonin: the doses studied when it clearly works are tiny. The classic experiments used about 0.3 milligrams, enough to raise blood melatonin only to a normal night's level. In one study of older adults, that small physiological dose restored sleep efficiency, while a dose ten times larger worked no better and left melatonin elevated into the next day, a signal that can read as morning grogginess. The researchers put it plainly: less is more.

Yet most products carry three, five, even ten milligrams, many times what the science used, and more is not a stronger version of the same effect. The printed number may not be real either: when one analysis tested thirty-one supplements, the actual melatonin ran from about 83 percent below to nearly 478 percent above the label, and a quarter contained serotonin entirely unlisted. Sold as a dietary supplement, melatonin is not tested the way a medicine is.

So what is melatonin, honestly? A timing signal you can borrow, not a sedative you can lean on. It earns its keep when your clock and your world disagree, on the far side of a red-eye or at the front of a shift that starts before dawn. For ordinary restlessness it offers a few minutes, not a rescue, and at a small dose taken at the right time, because melatonin is about when, not how much.
