Free Testosterone, the Fraction That Actually Does the Work

A normal total testosterone result does not always mean the body has enough hormone it can use. A calm look at why the free fraction, not the total, often explains the symptoms, and why the two numbers can tell different stories.

A blood draw comes back. The testosterone line reads normal. And yet the fatigue, the flat libido, the slower recovery in the gym, none of it lifts. This is one of the more common gaps in hormone testing, and it usually comes down to a detail the report rarely explains: total testosterone and free testosterone are not the same measurement, and only one of them reflects what the body can actually use.

Two numbers, one hormone

Testosterone travels through the bloodstream almost entirely chaperoned. Most of it rides bound to proteins, principally sex hormone-binding globulin (SHBG) and, more loosely, albumin. Bound hormone cannot dock with a receptor and do its job; it is cargo in transit, not yet active. A total testosterone test adds up everything in the tube, bound and unbound together, without distinguishing one from the other.

Free testosterone is the fraction with no protein attached at all, unbound and immediately available to enter tissue and act. It is a small slice of the total, generally cited as somewhere around 2 to 3 percent, with the rest held by SHBG and albumin. A third test, the bioavailable testosterone test, tries to capture a middle ground: the free hormone plus the portion loosely bound to albumin, which unbinds easily enough to still count as usable. It is not commonly run.

Total testosterone is the standard first test for a reason. It is inexpensive, widely available, and it is what most routine panels report. But because it does not separate active from inactive hormone, a number in the "normal" range can still hide a shortage of what the tissues can actually reach.

Why the split matters

The variable that decides how much of a given total ends up free is SHBG. Raise SHBG and more testosterone gets locked away, even if total production hasn't changed. Lower SHBG and more of the same total testosterone is set loose. That is why two people can post an identical total testosterone result and feel entirely different: one may have low SHBG and plenty of active hormone, while the other has high SHBG and a much smaller usable fraction, despite matching numbers on paper.

SHBG itself moves with the rest of physiology, which is what makes free testosterone worth checking rather than assuming. Insulin resistance and higher body weight tend to push SHBG down, so a man with those conditions can show a lower total testosterone that still leaves adequate free hormone in circulation. SHBG also shifts with age, which means the usable fraction can drift over the years even when the total number looks steady, as a changing share of a similar-looking pool gets locked away or set loose.

How a diagnosis is actually built

The American Urological Association's guideline on testosterone deficiency is explicit that this is not a one-number decision. Clinicians are directed to use a total testosterone level below 300 ng/dL as a reasonable threshold, but a diagnosis requires two separate measurements, both drawn in the early morning, when levels are naturally highest, on different days. Even then, a low number by itself isn't the diagnosis. The guideline states that testosterone deficiency is only confirmed when a low total testosterone level appears alongside real symptoms or signs, such as low libido, erectile difficulty, reduced muscle mass, or depressed mood.

Once low testosterone is on the table, the guideline calls for follow-up testing beyond the hormone itself: luteinizing hormone (LH), to help sort out whether the signal is originating in the testicles or further upstream in the pituitary; prolactin, when LH is also low; and, in some cases, estradiol. Free testosterone earns its place specifically when the total result doesn't match the clinical picture, when SHBG is suspected to be abnormal, or when someone is already on testosterone therapy and a clinician wants to confirm the dose is actually landing.

When the free number is the one that explains things

The pattern shows up often enough to have a name in practice: a man with symptoms consistent with low testosterone gets a total testosterone result that reads normal, and the story ends there unless someone checks further. If SHBG happens to be elevated, that same total can be sitting on top of a free testosterone level well below where it should be. Checking free testosterone, sometimes measured directly and sometimes assessed alongside total testosterone, SHBG, and albumin on a single panel, is what surfaces the mismatch.

This cuts both ways. A total testosterone result flagged as low does not automatically mean someone is testosterone deficient either. If SHBG is unusually low, as it can be with obesity or insulin resistance, a below-range total can still be accompanied by a perfectly adequate free fraction. Reading either number in isolation risks either missing a real deficiency or manufacturing one that isn't there.

None of this makes total testosterone testing pointless. As a screen, it remains the reasonable first step for most people, and it is what most reference ranges and most insurance-covered panels are built around. Free testosterone is the second question, asked when the first answer doesn't line up with how someone feels, or when a condition known to shift SHBG is already part of the picture.

Is free testosterone always better to test than total testosterone?
